Most ED pills work on one thing: blood flow. But 52% of men over 40 experience ED1 – and for many, the problem isn’t just physical.2 Stress, anxiety, and confidence all play a role.
When our 42-year-old tester (we'll call him Ryan) started looking for something more effective than the Cialis prescribed by his doctor, he found plenty of online treatments promising to outperform traditional options. But what actually works - and why?
We tested and reviewed four of the most popular online ED treatments - Helmd Drive, BlueChew Gold, Hims Hard Mints, and Ro Sparks. And Dr. Joshua Elmore (MD), a primary care physician who treats men's ED, lends his expert medical opinion. We found one clear winner - read on to find out which.

What to Look for in an ED Treatment
An erection depends on two systems working together: blood flow to the penis, and the right arousal signals firing in the brain. Most ED medications only address blood flow.
Drugs like sildenafil (the active ingredient in Viagra) and tadalafil (Cialis) are PDE5 inhibitors - they block an enzyme that constricts blood vessels, allowing more blood to flow. The result is "harder, longer-lasting erections,” says Dr. Elmore.
But blood flow is only half the equation. “To get an erection, your brain must fire signals to your penis - and these drugs do nothing for the brain,” Dr. Elmore says. Performance anxiety, stress, and distraction all contribute to ED. That's why the most effective treatments also include ingredients proven to boost arousal signals in the brain. And clinical backing matters: unproven ingredients or formulation combinations can introduce side effect risks.
In short: the best ED treatments support both physical and mental arousal, work quickly, and have clinically-validated formulations. Here's how Helmd Drive, BlueChew Gold, Hims Hard Mints, and Ro Sparks compare.
ED Treatments Compared

Formulation & Side Effects:
Helmd Drive Wins With Only Clinically-Proven Ingredients

All four treatments we reviewed include sildenafil and tadalafil - PDE5 inhibitors that increase physical arousal. But the formulations diverge significantly from there.
Helmd Drive and BlueChew Gold are the only two that also include apomorphine, a dopamine agonist clinically proven to amplify arousal signals in the brain. In a study of 5,000 men, apomorphine led to a 103% increase in erections compared to baseline without it.3 "Dopamine is the 'feel good' chemical," explains Dr. Elmore. "Apomorphine improves sensation, pleasure, and mood." Hims Hard Mints and Ro Sparks contain no apomorphine - and Hims adds vitamin B12, marketed as "extra support," despite no clinical evidence linking B12 to improved erectile function.4
BlueChew Gold adds a fourth ingredient: oxytocin. A 2021 study found no improvement in sexual function after oxytocin administration,5 and oxytocin introduces risk of more severe side effects including blood pressure changes, nausea, and fluid retention.6 There is also no published research on how oxytocin behaves when compounded with three other active ED medications simultaneously - high risk, for no proven benefit.
Side effects for the other meds are mild and rare. Sildenafil and tadalafil - present in all four - have very low rates of mild side effects like flushing and indigestion.⁷,⁸ Apomorphine is also well-tolerated with only mild side effects in a small percentage of users.3
Helmd Drive wins with the only formulation that supports body and brain with only clinically-proven ingredients - no unproven combinations.
Effectiveness:
Helmd Drive Wins With The Most Complete Experience

In an ED treatment, two things matter most: speed to onset and staying power. Helmd Drive, Ro Sparks, and BlueChew Gold are all sublingual - they dissolve under the tongue and kick in within 15 minutes. Hims Hard Mints are chewable tablets that pass through the digestive system, taking 30-60 minutes. All four last up to 36 hours once active.
What sets Drive apart is how complete the experience feels. Within 15 minutes, Ryan felt "in the mood and ready to go," which translated into "more confident and present sex" than with any of the other options. Ro worked on a similar timeline but felt "more mechanical" to Ryan - effective physically, but missing the mental component. Hims “took too long to kick in” and provided “no mental boost.”
Ryan noticed mild nausea shortly after taking BlueChew Gold - likely from the oxytocin. That made the experience less satisfying, and offset the mental benefits of the apomorphine.
Bottom line: Drive was the quickest to quick in and the most complete experience.
Convenience & Discretion:
It's a Tie Between Helmd Drive & Ro Sparks

Discretion is important for ED meds - on a date or in the bedroom, you don’t want to be advertising a pill or having to time doses.
Helmd Drive and Ro Sparks are the most discreet options in our test. Both are sublingual tablets that dissolve under the tongue in about 5 minutes - easy to take without raising eyebrows. Helmd Drive has a faintly minty, mild taste, while Ro Sparks is, in Ryan’s words, “spicier and more medicinal.”
BlueChew Gold is also sublingual, but the berry flavor is overly medicinal - “gross and hard to tolerate while the tablet dissolves,” says Ryan. It took 10 minutes to dissolve and left a blue tint on Ryan’s tongue - not the most discreet. Hims Hard Mints are chewables and much easier to spot.
Price & Value:
Helmd Drive Wins with the Most Affordable Formulation

How does cost per dose compare?
Helmd Drive is the clear winner - starting at $5 per dose, it's the most affordable in the group. Ro Sparks is the priciest at $12 per dose, while Hims Hard Mints and BlueChew Gold start at $7/dose.
Considering Drive's faster onset, complete formulation, and clean side effect profile, it's the best value - especially for men who haven't had full success with standard treatments alone.
The Verdict: Helmd Drive Wins Our ED Med Test
Which ED treatment is most effective? After testing all four options, we recommend the one that works in 15 minutes, supports both physical blood flow and mental arousal, comes in a discreet sublingual tablet, and is affordable for regular use. And that's Helmd Drive.
Dr. Elmore agrees: "I recommend Helmd Drive. It's a clinically supported ED treatment that addresses both the physical and mental sides of sex.”
