And Dr. Robert Woodward (MD), a primary care physician who treats men's sexual health, lends his expert medical opinion. We found one clear winner - read on to find out which.

What to Look For In a PE Treatment

PE comes down to two things: how your body regulates the ejaculation reflex, and what's happening in your head. “Serotonin is one of the major neurotransmitters involved in regulating ejaculation. For many, PE is also tied to stress, performance anxiety, or another issue like erectile dysfunction (ED),” says Dr. Woodward.

That's why the most effective treatments increase serotonin in the brain to delay ejaculation. Selective Serotonin Reuptake Inhibitors (SSRIs) like paroxetine and fluoxetine boost serotonin in the brain, spinal cord, and peripheral nerves, enhancing serotonergic signaling in pathways involved in ejaculation and delaying climax.

There's also the erection side of things. “A lot of men with PE are also dealing with some degree of ED - trouble getting or keeping an erection firm enough for sex," says Dr. Woodward. ED commonly coexists with PE, with studies reporting substantial overlap between the two conditions. In fact, ED is a factor in up to 50% of PE cases.4

Both Rugiet Go Long and Hims Sex Rx + Climax Control take a dual approach to PE. Each pairs an SSRI - which raises serotonin to delay climax - with tadalafil, a PDE5 inhibitor that improves blood flow to the penis and supports erections. In other words - both target PE and the ED that often accompanies it.

But Rugiet Go Long is the only treatment that contains paroxetine - the SSRI with the strongest clinical evidence in treatment of PE. In multiple trials, paroxetine delayed ejaculation more than any other SSRI, increasing time to climax 800% over baseline.5 Hims Sex Rx + Climax Control uses fluoxetine, which has weaker clinical evidence for PE.

“Combining paroxetine and tadalafil produces a synergistic response - the two together work better than either could alone,” says Dr. Woodward. “Paroxetine increases time to ejaculation, while tadalafil improves erections.”

Roman Swipes are in a different category: a 4% benzocaine wipe applied to the penis directly, with no internal mechanism and no mental support. It can delay climax in mild PE cases by numbing the surface, but it does nothing to address the serotonin signaling that causes PE - and has the downside of reducing pleasure sensation. They’re also awkward to apply just before sex, and leave a bitter taste on the penis.

In a PE treatment, men need two things: climax delay and stronger, longer-lasting erections. Rugiet Go Long and Hims Sex Rx + Climax Control both work systemically - an SSRI to delay climax, plus tadalafil for erections - while Roman Swipes work topically, numbing to slow things down. 

Rugiet Go Long is taken at need - each dose should be taken 1-4 hours before sex, and will stay effective for up to 36 hours. Hims is a daily pill that’s meant to improve time-to-climax whenever you have sex. Roman should be applied directly to the penis 5 minutes before the encounter, and the numbing effect lasts only 20-30 minutes after application. 

What sets Rugiet apart is how complete the experience feels. According to Michael, within a few hours of his first dose he felt “in control without feeling checked out,” which translated into “more relaxed, confident sex - and I still felt everything.” With Hims, on the other hand, Michael felt the daily SSRI “helped improve my control and confidence a bit,” but the improvement “wasn’t as noticeable as Rugiet” - and he noticed emotional numbing from the daily SSRI exposure. Roman “was a mood killer in the moment,” and Michael “barely noticed any improvement.” If you only suffer from a minor case of PE, consider Roman - but otherwise skip the topical wipe.

Rugiet Go Long stands out as the most flexible option. It's an on-demand pill taken 1-4 hours before sex, with a 36-hour window - so you take it well before you plan to have sex, and it lasts the whole weekend.

Hims Sex Rx + Climax Control is a traditional daily pill, which means continuous SSRI exposure - whether or not you plan to have sex. As a single guy, Michael would rather not commit to a daily SSRI. Roman Swipes need a 5-minute dry time, are single-use, and carry partner-transfer risk. As Dr. Woodward puts it, "numbing agents work by taking away the sensations of sex, and can transfer to a partner or even contribute to ED over time.”6 Pulling out a wipe just before sex can be awkward and embarrassing.  

Michael experienced no side effects on Rugiet during testing. Side effects of on-demand paroxetine and tadalafil are mild and rare - think headache or upset stomach.7 

Continuous SSRI exposure, on the other hand - like with Hims - carries risk of more severe side effects like “lowered libido and emotional blunting,” says Dr. Woodward, as well as nausea, weight gain, and vision changes.8

Both pills feature similar mechanisms, but Rugiet is our preference for this drug combo. A stronger serotonin reuptake inhibitor that you only need to use on-demand. That means flexibility, and fewer side effects than with a daily SSRI.
